These are the terms that apply when you use Form. Plain English; same voice as the app.
Form is a software product for tracking food and training. It is not medical advice, not a personal trainer, not a substitute for a doctor, dietitian, or physical therapist. If you have a medical condition, are pregnant, are recovering from an injury, or are about to start a new diet or training program, talk to a qualified professional first.
You can: use Form for personal, non-commercial purposes; export your data; cancel any time.
You can't: reverse-engineer the app or its APIs; resell access; use Form to harm others or violate any law; submit content you don't have the rights to (e.g. photos of other people without consent).
Form uses third-party AI (Anthropic Claude) to estimate macros from text/photos and to return form-check observations. These estimates are best guesses with explicit uncertainty. They will sometimes be wrong. Verify anything important — especially calorie counts for medical purposes — against a trusted source.
Form-check is not a substitute for a qualified coach. If a movement causes pain, stop.
Form is free to use. There are no in-app purchases and no subscriptions.
How we handle your data is documented in the Privacy Policy. By using Form you confirm you've read it.
You own your data. We just store it for you.
We aim for high availability but we don't guarantee uninterrupted service. Form depends on third-party providers (Supabase, Anthropic) whose outages affect us. No service credits are offered for downtime.
You can delete your account from inside the app at any time (Settings → Danger zone). We can terminate accounts that violate these terms.
When an account is deleted, all data tied to it is wiped from our active database. Backups roll off within 30 days.
Form is provided "as is" without warranty of any kind. We disclaim all implied warranties of merchantability, fitness for a particular purpose, and non-infringement to the maximum extent permitted by law.
To the maximum extent permitted by law, our total liability to you for any claim related to Form will not exceed the greater of (a) the amount you paid us in the 12 months preceding the claim or (b) US$50.
These terms are governed by the laws of the State of Texas, USA, without regard to conflict-of-law rules. Any dispute will be resolved in the state or federal courts located in Travis County, Texas.
We may update these terms; material changes will be announced in-app at least 14 days before they take effect.
